Immunization Clinics

Immunizations are available by appointment each Thursday.  For appointments, call 385-3831.  
Records of all previous immunizations are required.

Parents are responsible for informing schools and/or Doctor's Office of child's shots.

There is an $8.00 per injection administration fee or bring your Hoosier HealthWise card with you.
Walk-ins will be seen in a timely manner.

Tuberculosis (TB) Testing

A $10.00 administration fee is required for each test performed.  
Testing is done at the Health Dept
